Residents of the Redlands and Bayside can learn how to plan for their future health care by attending free advance care planning (ACP) clinics during 2024.
The clinics, hosted by the Metro South Health ACP Service team and held monthly at Wynnum-Manly Community Health Centre, Gundu Pa and the Redlands Satellite Hospital, will enable consumers to learn more about ACP and receive support to complete Queensland ACP documents including the Enduring Power of Attorney, Advance Health Directive and Statement of Choices.
Christine Duncan, Nurse Unit Manager of the Metro South Health ACP Service team says that ACP supports people of any age or stage of health to plan their health care, in advance, by sharing their personal values, goals and health care preferences.
“Planning ahead can help to ensure that the treatment and care you receive in the future aligns with your wishes,” Christine says.
“It also means that your loved ones won’t have to make difficult health care decisions on your behalf without knowing what you would have wanted. Your words can guide those making decisions about your health care when you can’t speak for yourself.”
Experienced ACP Clinical Nurse Consultants facilitate the clinics and can assist consumers and their families to understand the types of health care decisions that may be needed and support the completion of ACP documents.
